As an Independent Living Center, ARISE does everything it can to ensure that people with disabilities can live safely and successfully in the community. We offer  different programs with that goal in mind. 

Open Doors Transition Center / Money Follows the Person

ARISE’s Open Doors / Money Follows the Person program helps residents of long-term care facilities who want to move back to their homes or communities. Our trained Transition Specialists and Peer Advocates are experts in community resources and supports. We work with each resident to connect them to appropriate services for a smooth, safe transition from a facility to the community.  

Counties served:  

Onondaga, Oneida, Herkimer, Jefferson, Lewis, St. Lawrence, Oswego, Madison, Cayuga.

Regional Resource Development Centers (RRDC) 

Through its Nursing Home Transition and Diversion program (NHTD), the RRDC helps people receive services in the community instead of an institution .  Our trained Regional Resource Development Specialists will help qualified individuals access and manage services from approved providers.

Onondaga, Oneida, Herkimer, Jefferson, Lewis, St. Lawrence, Oswego, Madison.
