Are you confused by how to set up the services you need to continue living in your home? Do you know someone who would like to move from an institution into the community?
ARISE’s Health Care Advocate works to protect the rights of people with disabilities so they can live independently in their community. Our Health Care Advocate helps you understand your rights and will guide you in:
- Setting up services, such as home health care, Meals on Wheels, transportation, home modifications, etc.
- Finding the doctor or specialist you need who accepts Medicaid
- Finding vendors who sell the durable medical equipment you need
- Accessing other support services you need, such as case management and Medicaid Service Coordination
- Moving from an institution into the community
- Providing referrals for appropriate housing
Our Health Care Advocate also provides technical assistance to help professionals ensure their health care facilities are fully accessible.
