Community Habilitation Services

Do you need assistance to increase your independent living skills? Would you like to get more involved in your community?

ARISE's Community Habilitation Services offer a person-centered approach to helping people who have a developmental disability increase independent living skills and community involvement.

Your Community Habilitation Coordinator works with you to develop an individualized habilitation plan designed to help you achieve your personal goals. You can have services one-on-one or in small groups up to 4 people, as you develop, maintain, or increase independent living skills such as:

Household tasks such as cooking or laundry
Health and personal safety

Services are provided in your home or the community, depending on your interests and needs.
