Personal Care Assistant (PCA)

Job Summary:  The purpose of this position is to provide personal care assistance as directed by the consumer in order for the consumer to live as independently as possible.  This incumbent will perform a variety of tasks which may include personal care, housekeeping, grocery shopping, meal preparation, etc.


Essential Functions:

  • Follow written and/or verbal instructions, according to Consumer’s Care Plan
  • Examples of tasks may include, but are not limited to: assisting the consumer with transferring in/out of the shower, assisting the consumer with transferring to bed, toilet, shower and wheelchair, using appropriate medical equipment (i.e Hoyer lift, Stand to sit, etc.) as necessary to ensure a safe transfer, shaving the consumer, taking out the trash, dressing/undressing the consumer, light housekeeping (kitchen, bedroom, bathroom and living room), making the bed/change the linens, washing, drying and folding laundry, assisting with meal preparation, shopping for groceries, completing errands as approved by ARISE and the insurance company, toileting assistance as needed (may include changing briefs, and/or emptying and cleaning a urinal, and/or emptying and changing a catheter bag), assisting with range of motion exercises, feeding/setup Breakfast, Lunch, Dinner and positioning/turning the consumer.
  • Complete and submit to CDPAP Manager service documentation / time sheets every Monday by noon
  • Adhere to call-in procedures (“MyTime” system)
  • Attend all mandatory meetings (trainings, program updates, etc.)
  • Perform all tasks in the Care Plan including  physical tasks such as: lifting,  carrying (grocery bags, for example); reaching; kneeling; stooping; bending; etc., depending upon the consumer’s need for assistance
  • Uphold the codes of conduct and ethics as well as all ARISE policies and procedures
  • Exercise sound judgment as a matter of course and hold protected health information and other sensitive information in the strictest of confidence in accordance with ARISE and HIPAA policies
  • Other duties as assigned




  • Must have a working telephone number, or a phone to relay messages in a timely manner, at all times
  • Promptly report any address, phone number and/or name changes to ARISE CDPAP Manager and/or Human Resources
  • Maintain a clean driver license, if driving is required to perform job duties
  • Maintain compliance with annual physical requirements per NYS DOH
  • Clear drug screen and  background checks as required
