Job Title:        Consumer Directed Personal Assistance Program (CDPAP) Coordinator, Grade 5

Reports to:    Manager of CDPAP

FLSA Status:  Non-exempt  

Job Summary: The CDPAP Coordinator processes and maintains the fiscal needs of individuals, previously authorized by their Medicaid or Managed Care Company to receive in home services through the Consumer Directed Program. The incumbent also guides clients in the supervision of the personal assistant/employee who assists the individual and maintains contact for ongoing feedback and quality improvement.

Essential Functions: 

  • Monitor, maintain and audit client files and authorizations to ensure quality improvement and continuous coverage
  • Maintain regular communication with insurance companies to update records, resolve discrepancies and inform company representatives of Consumer Directed Program policies and procedures
  • Communicate regularly with the agency’s Human Resources and Billing Departments to ensure accurate representation of department operations
  • Meet with and educate potential consumers and their circle of supports to inform them about program services and assess the individuals’ needs to make appropriate program recommendations
  • Maintain contact (in-person, phone, US mail and/or e-mail correspondence) with individuals and families in accordance with and to promote Consumer Directed Program regulations, Quality Improvement, Corporate Compliance and the ARISE Code of Conduct
  • Maintain professional demeanor while assisting with consumer and/or Personal Assistant crises or incidents. Accurately document and report information in a timely manner to management and/or Quality Improvement
  • Provide guidance to Personal Assistants by assisting in their recruitment, overseeing accurate and timely service documentation submissions, conducting trainings as necessary and resolving employee concerns
  • Attend and participate in interdepartmental meetings, meetings with funding representatives and out-of-town networking conferences
  • Schedule intakes and annual home visits
  • Complete accuracy verification through monthly reporting (service documentation, billing and payroll audits, personal assistant performance evaluations and annual progress notes in the agency database)
  • Assist with program marketing presentations/materials and updating CDPAP Participant Manual
  • Uphold the codes of conduct and ethics, as well as all ARISE policies and procedures
  • Exercise sound judgment as a matter of course, and hold protected health information and other sensitive information in the strictest of confidence in accordance with ARISE and HIPAA policies
  • Other duties as assigned


  • Bachelor’s degree in human services or related field preferred and two years supervisory experience
  • Excellent verbal/written communications, organizational and record keeping skills
  • Working knowledge of billing and payroll procedures
  • Proficient in Microsoft Word and Excel
  • Clean driver license and reliable transportation
  • Familiarity with home health care services and regulations
