Job Title: Quality Improvement Specialist
Reports to: Director of QA, Contract Management & Corporate Compliance
FLSA Status: Non-exempt
Summary: The purpose of this position is to support ARISE’s continuous quality improvement initiatives in the areas of incident investigations and program quality assurance. As a result, ARISE will increase its efficiency regarding implementation and monitoring of corrective plans, staff training and adherence to regulatory and funding requirements.
Essential Functions:
- Conduct agency-wide incident investigations including:
Preliminary and Final Investigation Reports;
On-site follow-up interviews as needed;
Present investigation reports to the ARISE Incident Review Committee;
Conduct in-person and phone interviews of consumers, family members - Accept direction from QA Director and work collaboratively with Program Coordinators and Department Managers regarding reporting incidents, follow-ups and plans for protective and/or corrective action
- Collaborate with Service Coordinators and other outside agency providers
- Assist with internal programmatic audits/reviews
- Assist with providing community observational monitoring of service deliverables
- Conduct field visits of direct care staff and provide on the job training when necessary
- Monthly reports- Evaluations; assessments, audits
- Review employee service documentation for Medicaid compliance and provide education/training/feedback when necessary
- Attend required workshops and trainings
- Monitor consumer satisfaction through phone assessment and program evaluations
- Other duties as assigned
Qualifications:
- Bachelor’s degree or equivalent combination of education and experience
- -3 years experience working with people with developmental disabilities; or, one year experience as a Service Coordinator, Day or Residential Habilitation program coordinator
- Attendance at NYS Office of Mental Retardation and Developmental Disabilities (OMRDD) - approved Incident Reporting and Investigation Training programs (Basic through Advanced) within three months of commencing this assignment, unless certificate of recent prior training completion produced
- Knowledge of Onondaga/Madison/Oswego County human service agencies and resources
- Fifteen hours of professional development annually, prospectively
- Proficient in MS Excel & Word, familiar with Access
- Solid organization and record keeping skills
- Excellent communication (verbal and written) skills
- Previous QA/CQI and incident management experience preferred
