 Nurse Evaluator / Regional Resource Development Specialist

Reports to: Regional Resource Development Center Manager (RRDC Manager)

FLSA Status: Exempt

Job Summary:  The NE/RRDS must be an experienced professional with demonstrated skills and a dedicated commitment to work to support seniors and individuals with disabilities who wish to live in community-based settings.

Responsibilities:
The NE/RRDS is responsible for:

  • Completing the H/C-PRI and SCREEN for potential waiver participants, as needed
  • Conducting annual level of care (LOC) assessments for wavier participants as needed;
  • Managing and delivering the program objectives of conversion, repatriation, diversion, waiver administration and resource development;
  • Assisting the RRDC when requested with attracting quality new providers to offer needed services
  • Participating in local consortium and regional forums on issues involving individuals with disabilities and seniors relevant to the NHTD waiver;
  • Meeting with potential waiver participants, determining waiver eligibility, and offering individuals the choice in waiver program participation and Service Coordinators;
  • Reviewing, monitoring, and approving TBI/NHTD waiver participant Service Plans, especially those that are medically complex, that are developed by the waiver participant and the Service Coordinator
  • Reviewing revised plans every six months to monitor for quality and cost effectiveness;
  • Interviewing potential waiver providers and make recommendations to DOH on whether to enroll them as providers based on factors such as regional need or the provider’s ability to deliver quality, cost-effective services to the population;
  • Providing training and technical assistance to waiver service providers on all aspects of the NHTD and TBI Waivers;
  • Working closely with DOH to provide data and input as needed regarding all aspects of the administration of the waiver in the region;
  • Other duties as assigned by the RRDC Manager

Qualifications:

  • Licensure / Certifications
    1. a current New York State licensed registered professional nurse with two years’ satisfactory professional experience working with individuals with disabilities and/or seniors in a community setting; and
    2. New York State certified to complete the Hospital and Community Patient  Review Instrument (H/C-PRI) and SCREEN.
  • Working knowledge of services and supports available through Medicaid, Medicare and/or other third party payers
  • Excellent clinical assessment, screening, and interviewing skills
  • Organizational, problem-solving, and conflict resolution skills
  • Ability to coordinate activities, evaluate data, and establish priorities
  • Professional experience working with people with disabilities and/or seniors
  • Working knowledge of community-based resources for individuals with disabilities and/or seniors
  • Experience developing relationships with institutional and/or community-based organizations
  • Excellent written and verbal communication skills
  • Computer proficiency; Microsoft Word, Excel, WinZip
  • Clean driver license, reliable transportation
