 Lead Regional Resource Development Specialist/Nursing Home Transition & Diversion

Reports to:   Director, Independent Living Center
FLSA Status: Exempt

Summary
The Lead RRDS must be an experienced professional with demonstrated skills and a dedicated commitment to working to support seniors and individuals with disabilities who wish to live in community-based settings.

Qualifications
To qualify for the Lead RRRDS/NHTD position, an individual must possess one of the following:

  • a baccalaureate degree or higher in health or human services or a related field and four years of combined satisfactory professional experience in working with individuals with disabilities and/or seniors
  • be a Registered Professional Nurse and have four years of combined satisfactory professional experience working with individuals with disabilities and/or seniors, including in a managerial capacity
  • possess an Associate’s degree and ten years of combined satisfactory experience working with individuals with disabilities and/or seniors, including in a managerial capacity
  • possess an Associate’s degree and four years of satisfactory experience as a Regional Resource Development Specialist

To be qualified for the position of Lead RRDS, the individual must be an experienced professional with demonstrated skills and a dedicated commitment to working to support seniors and individuals with disabilities who wish to live in community based settings. The individual must also possess the following:

  • Experience supervising professional staff who work directly with seniors and individuals with disabilities
  • Knowledge of the operation of 1915 (c) waivers and community-based services
  • Working knowledge of services and supports available through Medicaid, Medicare and/or other third party payers
  • Working knowledge of community-based resources for individuals with disabilities and/or seniors
  • Experience developing relationships with institutional and/or community-based organizations
  • Excellent assessment, screening and interviewing skills
  • Organizational, problem-solving, and conflict resolution skills
  • Experience in program development and implementation
  • Ability to coordinate activities, evaluate data, and establish priorities
  • Excellent written and verbal communication skills
  • Computer proficiency; Microsoft Word, Excel, WinZip
  • Clean driver license, reliable transportation

Responsibilities
The Lead RRDS is responsible for:

  • Managing and delivering the program objectives of conversion, repatriation, diversion, waiver administration and resource development;
  • Assisting DOH in ensuring that Wavier Participants in New York State are able to live as independently as possible in the community.
  • Implementing the HCBS/NHTD waiver and improving access to NHTD program supports;
  • Assessing, identifying, and addressing gaps in services;
  • Developing and implementing a comprehensive outreach campaign to recruit Wavier Providers
  • Building NHTD community consortiums and developing NHTD provider competence;
  • Developing relationships with LDSS’s and other local and state entities, community based healthcare providers, and inpatient rehabilitation units and long term care facilities.
  • Meeting with potential waiver participants, determining waiver eligibility and offering individuals the choice of participation in the HCBS/NHTD waiver and a choice of waiver Service Coordinators;
  • Reviewing, monitoring, and approving HCBS/NHTD waiver participant Service Plans that are developed by the waiver participant and the Service Coordinator, and reviewing revised plans every six months to monitor for quality and cost effectiveness;
  • Interviewing potential waiver providers and making recommendations to DOH on whether to enroll them as providers based on factors such as regional need or the provider’s ability to deliver quality, cost-effective services to the NHTD population;
  • Providing training and technical assistance to waiver service providers on the needs of the individuals with disabilities and how to develop waiver Service Plans;
  • Reviewing all information and investigations regarding Serious Reportable Incidents in accordance with the Serious Reportable Incident policy.
  • Working closely with DOH to provide data and input as needed regarding all aspects of the administration of the HCBS/NHTD waiver in their region; and
  • Other duties as assigned
