Community Integration Specialist, P/T - Onondaga
FLSA Status: Non-exempt
Reports to: Coordinator, Group Day Habilitation Program (site-based)
Job Summary: Responsible for providing a variety of habilitation services including case management and Individual Service Environment (ISE) planning with the individual's team. Provide specific observational assessment, training and support services to assigned consumers. Develop and implement services that will meet the individual’s needs per their ISE. Successfully integrate, to the greatest extent possible, the consumer into the community.
ESSENTIAL JOB FUNCTIONS:
- Provide direct services in the community that address identified needs and assist the individual in developing and maintaining more independence in the community.
- Develop services that will improve consumer functioning in areas where specific needs have been identified.
- Develop ISE plans with specific goals and activities that will meet day habilitation goals. Coordinate plan implementation with the individual's designated team.
- Function as an interdisciplinary team member to focus on issues related to providing services that will increase the consumers community functioning,
integration, awareness and ability to make choices. - Recommend services that will enhance the individual's ability to be more independent and integrated within the community.
- Assume case management functions for assigned day habilitation consumers. Complete related reporting, tracking and statistical information reports.
- Meet regularly with immediate supervisor to review cases and ISE plans and discuss issues related to day habilitation case management.
- Operate telephone, computer, and printer, photocopier and fax machine.
- Able to operate a motor vehicle and transport consumers safely
- Able to stoop, kneel, reach, sit, stand, walk, push, pull, lift, talk and hear.
- Able to exert up to 50 pounds of force occasionally and/or up to 20 pounds of force frequently, and/or up to 10 pounds of force constantly to move objects and assist people in and out of wheelchairs.
- Able to assist with personal care, including transferring and toileting as needed.
- Moderate mental and visual application needed to perform work dealing primarily with preparing and analyzing data and figures, computer, and necessary reading.
- Maintain professional conduct / detachment from personal involvements with the consumer(s) and / or their family members
REQUIREMENTS:
- Must successfully complete pre-employment screenings, including criminal history background check as directed by funder (OPWDD Criminal Background Check)
- High school diploma or equivalent with relevant work experience preferred.
- Clean, valid New York State driver's license acceptable to agency’s insurer
- Must be at least 21 years old, in order to meet agency’s insurer’s minimum age requirement to operate the program’s extended van
- Must have registered & inspected vehicle, with appropriate auto insurance coverage, so as to transport consumer as required
